General Purpose

Under the supervision of the Program Director, the primary focus of the Community Resource Specialist will be to provide supported living services for adults with developmental disabilities who live independently within their community in the living arrangements of their choice.

The Community Resource Specialist will conduct community outreach and provide assistance to those served. They will be part of a team, including the consumer's circle of support, to help meet the consumers' Individual Program Plan (IPP). The CRS will provide services including supervision, assistance, and support in areas outlined by the IPP.

The Community Resource Specialist will uphold People Services, Inc. core values of Respect, Dignity, and Empowerment in all interactions with consumers, team members, agency partners, and the community.

Qualifications

  • High School Graduate or equivalent
  • Experience working with people with developmental disabilities or related fields is strongly preferred.
  • Fingerprint Clearance
  • Negative Results to Drug Testing
  • Adherence to Drug Testing Policy
  • Valid California Driver's License
  • Clean Driving Record
  • CPR and First Aid Certification or the ability to obtain certification
  • Ability to obtain Pro Act Certification
  • Reliable, Registered, and Insured Vehicle
  • Ability to read and interpret documents, write specific correspondence, and interpret written and verbal instructions
  • Flexible schedule to meet the needs of the consumer

Essential Job Duties

Under the supervision of the Director, the Community Resource Specialist will:

  • Develop and maintain effective communication with the Program Director.
  • Prepare and monitor IPP for assigned clients, including training, coverage, and data collection.
  • Assist consumers with identifying personal goals; supported living services include money management, social security assistance, medical appointments, health insurance, peer support, self-advocacy, and socialization.
  • Assist consumers with housing issues, including obtaining accessible housing, accessing funding, communicating with landlords and housing agencies, and advocacy.
  • Help consumers achieve personal, behavioral, or independent goals as outlined in the IPP.
  • Help consumers understand their rights and responsibilities.
  • Assist consumers in understanding their choices and possible consequences related to their health and well-being.
  • Address challenging behaviors proactively and respectfully.
  • Focus on the person served, demonstrating flexibility, creativity, and commitment.
  • Respect the values of the person served and facilitate their expression of choices.
  • Provide advocacy when the person's preferences or needs are overlooked.
  • Develop respectful, trusting relationships while maintaining professional boundaries.
  • Maintain confidentiality of all consumer information and facility operations.
  • Submit operational paperwork, timecards, mileage sheets, and contact logs as per schedule.
  • Assist and ensure consumer safety at home and in the community.
  • Complete caseload reports timely.
  • Maintain current First Aid and CPR certifications.
  • Maintain Pro Act Certification as per current policies.
  • Participate in team and staff meetings.
  • Develop and maintain required written documents.
  • Make decisions aligned with People Services, Inc./Konocti Connections' mission, policies, procedures, and ethics.
  • Respect the consumer's chosen lifestyle.
  • Perform other duties as assigned by the Program Director.
