Service Coordinator - Bilingual (English/Hmong) Chico (Full Time)

Department: Case Management

Location: Chico, CA

Position: Bargaining Unit Position

Hours: Full Time (39 hours per week)

Salary & Benefits: $28.57 - $37.14 per hour. Complete package of fringe benefits including vacation and sick leave, health insurance, 17 paid holidays, and CalPERS retirement. Travel allowance for field work.

Responsibilities

• Provide case management services to an assigned group of persons with developmental disabilities.
• Develop and monitor Individual Program Plans.
• Coordinate services with other agencies and service providers.
• Conduct home visits with clients, families, or other care providers.
• Consult with clinical and other professional staff.
• Advocate on behalf of clients.
• Complete reports and forms.
• Maintain case records.
• Perform other duties as assigned.

Qualifications

• Equivalent to a B.A. degree in Social Welfare or one year of experience in a similar position.
• Fluency in English and Hmong (ability to speak, read, and write).
