LTC Community Team Lead

Monday-Friday 9a-5p some evenings and weekends

We are seeking a dedicated and experienced LTC Community Team Lead to oversee and coordinate our long-term care community operations. The ideal candidate will lead a team focused on delivering exceptional care and support to residents, ensuring compliance with regulatory standards, and fostering a positive and collaborative environment.
Key Responsibilities

  • Lead, mentor, and manage the LTC community team to ensure high-quality resident care and operational efficiency.
  • Coordinate daily activities and workflows within the community to meet organizational goals and resident needs.
  • Ensure compliance with all applicable long-term care regulations, policies, and procedures.
  • Collaborate with healthcare professionals, families, and external partners to enhance resident care and satisfaction.
  • Monitor and evaluate team performance, providing feedback and training opportunities to promote professional growth.
  • Assist in developing and implementing care plans tailored to individual resident needs.
  • Manage conflict resolution and address any concerns raised by residents, families, or staff promptly and effectively.
  • Prepare reports and maintain accurate documentation related to community operations and resident care.
  • Support continuous improvement initiatives to enhance service quality and operational processes.
Qualifications
  • Bachelor's degree in Healthcare Administration, Nursing, Social Work, or related field preferred.
  • Minimum of 3 years experience in long-term care or community healthcare settings, with at least 1 year in a leadership role.
  • Strong knowledge of long-term care regulations and best practices.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to manage multiple priorities and work effectively in a fast-paced environment.
  • Compassionate and resident-focused approach to care.
  • Proficient in Microsoft Office Suite and healthcare management software.
